Phone Consultations:
Make an Appointment with Dr. Moffat
First-time consultations take 90 minutes. Cancer patients may take a bit longer. The fee is $85.00 for the initial consultation. Supplements and shipping costs are extra. If you live outside of Idaho, there is no tax--you will have to take care of that yourself.
Any additional appointments such as rechecks are charged out at the rate of $65/hour. Supplements and shipping are in addition to this.

Payment it due, in full, at the time services are rendered.
Cancellations or rescheduling: If you don't call ahead more than 24 hours to cancel or reschedule, you will forfeit the $65. I only do 4 appointments each day and schedule my day around the clients.
